Your Primary Nurse or Therapist will develop an individualized plan of care for you and coordinate all of your services. A Multidisciplinary Team consisting of professional and paraprofessional staff will work closely with your Primary Nurse or Therapist to help you regain your health. In addition, your Primary Nurse or Therapist will assist with coordination of your insurance and supplies as well. All services will be provided in the comfort of your own home.
