The MetroWest Medical Center Transitional Care Unit (TCU) at Leonard Morse Hospital, is a 19-bed skilled nursing facility that cares for patients who who need additional inpatient care following hospitalization for surgery or a medical illness. The TCU staff works with patients, their families and physicians, to enable patients to return home.
TCU care is multidisciplinary. A team directed by a physician provides daily rehabilitation,skilled nursing and other care. The team consists of nurses, physical and occupational theapists, social workers, case managers, nutritionists, activities staff and other ancillary staff.
These professionals help patients to regain and maintain the level of activity and function that they had prior to undergoing surgery or becoming ill. In a goal-oriented yet therapeutic environment, patients are encouraged to wear comfortable, everyday clothing and participate in social and rehabilitative activities that promote mental and physical well-being. Treatment is family-centered, so family members can learn how to care for patients once they are discharged home from the TCU.
Fully equipped with a rehabilitation gym and activities areas, the TCU provides a casual, homelike environment designed to promote functional independence. TCU patients typically stay on the TCU 7 to 10 days before discharge home. The TCU admits patients from MWMC and other area hospitals, and accepts Medicare and most major health insurances.
