If you would like copies of your transcripts just print out this form and fill in all of the necessary information. Make sure to sign it and send it to the address stated with your check made out to MetroWest Medical Center. Each transcript copy, whether official or unofficial, costs $3.00. Your transcript request will be processed within 2 business days upon receipt.
