Hospice is a team-oriented approach to caring for individuals during the challenging journey of managing a life-limiting illness. Care is given to the individual wherever they prefer. It may be at home, in a nursing home or in an assistive living environment. Care is focused on maintaining comfort and dignity, not on curing. The interdisciplinary team caring for the patient and family consists of the following:

• Patient’s Personal Physician
• Hospice Medical Director
• Registered Nurses
• Home Health Aides
• Social Workers 
• Spiritual Counselor – Clergy
• Trained Volunteers
• Physical, Occupational and Speech Therapists, if needed

The team develops a care plan with the patient and family that includes expert pain and symptom management, emotional and spiritual support; as well as coordination of contracted specialty medications and assistive medical equipment. Nurses are available 24 hours per day to help meet the patient’s needs and support the family members and friends caring for the patient.

Spiritual Care
Spiritual care is offered to all patients, families and caregivers of hospice services. Our Spiritual Counselor is a minister by trade but is non denominational for hospice. The focus is on helping the patient with their spiritual needs and to assist them to find the peace that is needed to help them with their journey. Or if preferred, hospice will contact your own clergy and help arrange visits as needed.

Support Groups
Bereavement support groups are held three times a year, and are facilitated by our hospice licensed independent clinical social worker. These groups are free to the general public and meet once a week for eight weeks in the School of Nursing building at 85 Lincoln Street, Framingham. All who would like to consider attending a support group will need to meet with the social worker prior to the group’s start date. Start dates for the bereavement support groups are published in the local newspaper or can be obtained by contacting the Hospice Social Worker.

Bereavement Services
Bereavement services and counseling are provided to the surviving family and caregivers. These individuals are supported under bereavement services with phone and personal contact as well as mailings for thirteen months after the loved one is no longer with us. Also, twice a year hospice hosts a Memorial Observance for all patients cared for by the hospice program. All family and caregivers are invited to participate in this wonderful remembrance event.
