Palliative Care/Bridge Program
Bridge is a specialty type of care for individuals with a potentially life-limiting illness who are exploring palliative treatment for their illness. Care is provided by nurses with experience and expertise in pain and symptom management. These nurses also possess a unique understanding of some of the psychosocial and spiritual issues that these patients may experience. Bridge services are covered under the home care benefit of the patient’s insurance company.
Maternal Child Health
Our Maternal Child Health RN provides care and support to mothers both before and after the birth of their babies in the comfort of their own homes.
- Home visits for new Moms and babies
- Healthy Start Program
- Prenatal care for expectant Moms
The Maternal Child Health RN provides care to newborns and children with unresolved health issues.
- In-home neonatal and pediatric care
Private Care
This program is designed for patients who are interested in-home assistance that is not covered by their insurance. This may be in addition to the care provided under your insurance plan, or after your insurance coverage ends, or at any time when you feel that some additional in-home help would benefit you. Our nurses, therapists and home health aides will visit you in your home to assist you in meeting your health care needs.
